The Wisconsin Care Integration partnered with primary care clinics from September 2019 through August 2024 around the state to learn about the challenges that those clinics and their families face when they have a young child with developmental concerns. The aim was to build the capacity of the clinic to better serve families when a young child needed supports or resources to address developmental concerns. WiCII supported clinic staff to learn more about developmental and disability related resources through individual consultation and learning communities. WiCII also provided family navigation services to families. As one outcome of this work, the value of family navigation services to help families address their own barriers to needed services and supports for their child was deeply affirmed.
